If you are a former patient, or family member of a patient, of EAAA, we welcome your feedback about the care you received at the scene of your incident.
We are aware that you may have little or no recollection of your contact with us, but we would be very grateful if you could answer as many of the following questions as possible. This will enable us to review our work and ensure that we are giving the best possible care.
If you are a friend, family member or care giver and were at the scene of the incident, we would be grateful to hear your feedback too.
All feedback is passed on to the EAAA clinicians who attended your incident. It will also be available to the Care Quality Commission who regulate our service. This form below will take around 10 minutes to complete.
